2008-09 Forms

There are a number of forms used in the Choral Department.  Some are mailed directly to the parents; others are given to the students to bring home for parental signature and then return to Dr. Lambert.  For your convenience, and to ensure that copies are always available, we have included the forms on our website. All are in PDF format.  Should the need arise, please download and print any form that you need, fill it out completely, and return as requested.

Field Trip Parent Permission Form - This form is purposely generic so that it can be printed by families in an "emergency" situation, such as when the field trip is the next morning and the permission form was left in a locker or lost.
